Codeine adds slowed breathing on top of methadone's build-up, for very little extra effect. Do not combine. Keep naloxone nearby.
Additive respiratory depression on top of methadone's accumulating baseline, for very little added effect.
The risk here is to the body — breathing, heart, temperature or organs. Set limits before you start, because this kind of harm does not announce itself in time to react.
