Mitragynine has monoamine activity and MAOIs block the breakdown of exactly what it releases. Hypertensive and serotonergic reactions are both plausible. This includes ayahuasca and syrian rue, not only prescription MAOIs.
The risk here is to the body — breathing, heart, temperature or organs. Set limits before you start, because this kind of harm does not announce itself in time to react.
