Mixing Alcohol and Kratom

Kratom's ceiling on slowed breathing does not hold once alcohol is involved; heavy sedation and choking on vomit are the usual danger. Avoid this combination, and put anyone who passes out on their side.
Rated below heroin + alcohol, and the difference is real rather than cosmetic. Mitragynine is a partial mu-agonist that barely signals through the pathway driving opioid respiratory depression, so it has a ceiling that full agonists do not. Alcohol removes that ceiling by suppressing breathing through a separate GABA route the ceiling does not cover. The outcome is usually a hospital problem rather than a morgue one — ICU admission runs about 28% for kratom with alcohol against roughly 9% for kratom alone — but 'safer than heroin' is a very long way from safe. Heavy sedation, vomiting and aspiration are the common way this goes wrong.
The risk here is to the body — breathing, heart, temperature or organs. Set limits before you start, because this kind of harm does not announce itself in time to react.
